We are an outpatient care service in the south of Munich. Our young and dynamic team consists of nurses, nurses, alternative practitioners and health scientists. Our focus is on expert advice and care for our patients, on the cutting edge of research, in accordance with SGB V, XI and above all with heart. The confrontation with the need for care is a challenging experience – for those affected, their relatives but also the nursing staff. Tasks:
– Control of the patient-related care process through planning and evaluation of care
– Carrying out care in accordance with the latest nursing knowledge as well as legal and carrier-side requirements
– Ensuring the satisfaction of those in need of care
– Participation in the development, ensuring the implementation and further development of the care concept
– Participation in the implementation of care visits and ensuring the correctness of care classifications
– Management of the documentation system, combined with the transfer of information and observations

Qualifikation:
– Completed exam as a geriatric nurse or health and nurse
– Knowledge of primary care
– Empathy in dealing with patients and relatives
– Reliability and a hands-on mentality, you think in terms of solutions, not problems
– Two effective work modes: individual work and collaboration
– High sense of responsibility and flexibility
– Confident handling of common IT applications
– Very good spoken and written German and excellent communication skills
– Readiness for the shift system and weekend work
– Have a valid driver’s license
